REFERRAL INFORMATION
FOR CASE MANAGERS

REFER A CLIENT 

Heritage Homes Supportive Living accepts referrals from social service providers, case managers, and community organizations. 

Eligible participants may include individuals experiencing:

• Housing Instability

• Mental Health Challenges

• Substance Use Recovery

• Criminal Legal System Involvement

• Barrier to Securing Housing

REFERRAL PROCESS

1. Refer your client(s) by submitting our REFERRAL FORM 

2. Upon receiving a complete referral form, an INTAKE ASSESSMENT will be conducted by our staff

3. This is then followed up with a HOUSING PLACEMENT REVIEW

4. Lastly, the client(s) will receive notice of their appropriate PROGRAM PLACEMENT
